Service Description
Virtual consultation with a registered dietitian to complete nutritional plan if more screening was needed after the initial visit; to review and explain your nutritional plan in more detail; and to track your progress with your plan is and to make changes as needed to the plan.
Cancellation Policy
Before scheduling your first follow-up appointment, make sure you had met with a registered dietitian (RD) for an initial nutrition consultation. To schedule a follow-up appointment to meet with a RD, visit the “Book Now” page on the Lifelong Dietetics LLC website. The follow-up consultation is ~60 minutes long and occurs over Zoom video call. A confirmation email from Lifelong Dietetics LLC containing the Zoom meeting link will be sent to the client’s registered email address 24 hours prior to the scheduled appointment time. Before your appointment time, make sure to complete the Readiness to Change Questionnaire which can be found n the booking confirmation email. Clients may cancel or reschedule their appointment at least 24 hours prior to the scheduled appointment time without penalty. Cancellations or rescheduling requests made less than 24 hours before the appointment time, as well as missed appointments (no-shows), will incur a $55 late cancellation/no-show fee. Appointments can be cancelled or rescheduled online at any time. A valid credit card is required at the time of booking, and applicable late cancellation or no-show fees will be automatically charged if the 24-hour notice policy is not met.
