Initial Consultation

Cancellation Policy

All individual nutrition services begin with an initial consultation with a Registered Dietitian (RD). To schedule an appointment, please visit the “Book Now” page on the Lifelong Dietetics LLC website. After booking your appointment, a welcome packet will be sent to the email address provided on the booking form. The initial consultation is approximately 60 minutes in length. Clients are encouraged to complete all forms included in the welcome packet at least 24 hours prior to their first appointment tiime. If forms are not completed in advance, the nutrition assessment may extend into a subsequent appointment. All initial consultations are conducted virtually via Zoom. A confirmation email from Lifelong Dietetics LLC containing the Zoom meeting link will be sent to the client’s registered email address 24 hours prior to the scheduled appointment time. Clients may cancel or reschedule their appointment at least 24 hours prior to the scheduled appointment time without penalty. Cancellations or rescheduling requests made less than 24 hours before the appointment time, as well as missed appointments (no-shows), will incur a $55 late cancellation/no-show fee. Appointments can be cancelled or rescheduled online at any time. A valid credit card is required at the time of booking, and applicable late cancellation or no-show fees will be automatically charged if the 24-hour notice policy is not met.
